Annually, our district requires parents to review the Handbook for Students and Families and complete required forms.  This information is very important for our records. 

    This school year, the following forms are to be completed by parents/guardians :  All Handbook Forms, Emergency Health Information Card, Student Address Verification and PXU COVID-19 Parental Acknowledgement and Disclosure. 

    The PXU Handbook for Students and Families can be accessed electronically from the Students and Parents section of the PXU website and at the following link: https://www.pxu.org/Page/27772.  

    This year, an online tool has been developed to support parents with completing and submitting these forms electronically.  The handbook, acknowledgement forms and an instructional video can be accessed at: https://www.pxu.org/Page/27770.

     Please review the Handbook and submit completed forms electronically by Friday, March 5, 2021.  Verification documents that you are unable to scan electronically can be dropped off at the front of our school with student’s ID number in a designated box marked REGISTRATION.
