It's time to apply for admission!
To apply to Wilson College Prep, you must download and complete the documents listed below. All forms must be signed by the parent/guardian of the student.
- Wilson College Prep Application for Enrollments 24-25 School Year: English/Spanish
- Phoenix Union High School District Enrollment Form: English or Spanish
- Phoenix Union Open Enrollment Form: English/Spanish
- Freshman Class of 2028 Electives: English
- Residency Guidelines: English or Spanish
- Emergency Student Health Information: English/Spanish
- Home Language Survey: English or Spanish
You will also need to provide:
- Driver's license/passport for the parent/guardian
- Student Immunization record
- Student birth certificate
- Proof of current address
- Two emergency contacts for the Emergency Student Health Information
