How do I apply for a substitute certificate?

    • All first-time applicants must obtain an Arizona Fingerprint Clearance Card (plastic) issued by the Arizona Department of Public Safety. Call (602) 223-2279 to obtain a fingerprint packet and for more information visit our frequently asked questions. Fingerprint services are provided by your local law enforcement agency for a fee. Arizona Department of Public Safety charges a fee of $67.00 to process Fingerprint Clearance Cards and is payable by cashier's check or money order.
    • Complete an application for certification which may be downloaded. The original Fingerprint Clearance Card needs to be sent along with your Application for Certification; it will be returned. Submit official transcripts (bearing the original seal of the registrar). Photocopies will not be accepted. If the college/university prefers, transcripts may be mailed directly to the Arizona Department of Education-Certification unity. The Certification Unit will only accept degrees, programs, and courses from accredited institutions. Submit the correct fees by money order, cashiers check or personal check made payable to the Arizona Department of Education (ADE). Fees are not refundable and cash will not be accepted. Fingerprint services are available in the Phoenix area.
    • If verification of teaching experience is required, please submit a letter verifying the dates of employment and grade level taught. The letter must be on official letterhead and signed by the superintendent or personnel director.
